Gertrude Avis Forman (nee O'Rourke) age 86, of Avon died Friday morning at Avon Oaks Nursing Home in Avon following a long illness.
The family received friends Monday from 7-9Pm in the David Bogner Family Funeral Home 36625 Center Ridge Road North Ridgeville.
